This form is for us to look over and determine a good time for us to set up your in-person consultation. In-person consultations are preferred to ensure both Artist and Client are on the same page when booking your appointment.

A deposit will be required to confirm your appointment if your piece is selected. Deposit information is below.

    - Deposits are non-refundable so please be sure when booking that you are confident in wanting this tattoo and that you will be available for your appointment date.

    ​- Deposits are $100 - $200 ( depending on the piece you are inquiring about ). Your deposit will hold your appointment spot, and will come off of the final price of your tattoo.

    - Deposits may be transferred to a new appointment date only once, and only if a 24hr notice is given.

    - If you do not give a 24hr notice of cancellation it will result in forfeiture of your deposit.

    - If you arrive over 15 minutes late with no communication, it will be considered a no call no show, and result in forfeiture of your deposit.
